Remote Nurse Care Coordinator - ID-4841

Job Overview:

Insight Global is looking to hire a Telephonic RN for one of their Long Term Care clients.

You will be part of a remote team of clinicians that calls claimants or their families and facilities to conduct a short assessment by phone or zoom. In about 20 minutes, the RN collects and updates the claimant’s electronic health record, plan of care and provide a detailed note for any change note. You will be part of a remote team of clinicians that spends their days reviewing clinical assessments completed by field assessors and writing Plans of Care for Long Term Care Insurance and other government sponsored insurance plans.

Day to Day:

· Reviewing clinical assessments completed by field assessors to determine chronic illness status and develop a plan of care with supporting written documentation.

· Collaborate with field assessors and facility staff via phone to determine if further information is needed on a claimant or member’s health status

· Call claimants, family members or facility staff to update records on their present health status and provide a new plan of care, as well as handle telephonic assessments

· Work with new clients to develop processes that meet their insureds’ needs.

A Care Coordinator Case Manager must be caring and empathetic with customers. They must be able to work in a fast-paced environment and be very computer literate.

This position uses your eldercare expertise and excellent communication skills to assure that the company serves our clients with the highest degree of quality, respect, and customer service. The successful candidate must be caring and empathetic with claimants, members, and their families

Shift:

• Monday - Friday
• 9:30am to 6:00pm ETS

Required Skills:

• RN with compact license in good standing.
• 2-5 years of experience in case management, home care, hospice, and/or a rehabilitation setting with an aging population.
• Excellent computer, data input and technical skills including working with multiple screens
